Please mail a completed Medical Release form to PO Box 5034., Traverse City MI 49696 as soon as possible.  We will also need a copy of your child's birth certificate if he/she is new to TBAYS.

TBAYS will refund all but $25 up until the team's 1st game.  No refunds can be made after play has begun.
