Please mail a completed Medical Release form to 160 Hughes Dr., Traverse City MI 49686 as soon as possible. We will also need a copy of your child's birth certificate if he/she is new to TBAYS.
Registration fee is $158 for a full year ($138for U8). If you are playing fall only the fee is $100 ($85 for U8) Fee MUST be included with your registration. If you wish to pay by check, please use the printed registration form.

TBAYS Refund Policy
TBAYS Refund policy – We will refund payments for withdrawals made before the first game of each season. We are sorry but no refunds can be made after those dates.
